Details - Position is based in a home health post-acute care setting - Covers north-eastern Sacramento including Folsom area - Day shifts Monday through Friday, 8:30 AM to 5:00 PM - Average of four patient visits per day including evaluations and revisits - First to float as needed within the zone - Mileage reimbursement is provided per GSA guidelines - Uses Cerner EMR system Requirements - Active CA Occupational Therapist license required (license required if applicable) - Minimum 2 years of professional experience in a healthcare setting - Current BLS certification required - Current NPI number required - Valid driver's license and proof of automobile insurance required - Two professional references: one supervisor from the last 12 months and one peer or supervisor from within the last 3 years - Experience with Cerner EMR preferred - COTA with home health experience may be considered if no OT applicants Additional Information - Conduct evaluations and revisits for home health patients - Documentation of mileage and timesheets requires approval by home health supervisors - Attendance required at meetings twice a month for mileage/time documentation - Supportive multidisciplinary home health team environment - Orientation consists of 12 hours - First to float as needed within the assigned zone - Dress code and scrub requirements will follow home health standards American Traveler Job ID P-667685.
