Opportunity Information Sutter East Bay Medical Group (SEBMG) is recruiting for a Board Eligible or Board Certified Ambulatory Palliative Care Physician. Provide Palliative Medicine consultation services and participation in Palliative Supportive Care Services interdisciplinary team. Founded in 2005, Sutter East Bay Medical Group (SEBMG) is a rapidly growing multi-specialty medical group whose physicians and advanced practice clinicians work in more than ten care centers and four hospitals in the San Francisco East Bay Area. The foundation provides the latest technology and clinical support, allowing clinicians to focus on delivering exceptional patient care. Provide Palliative Medicine consultation services in multiple settings: Home, SNF, RCFE, Hospital and Office (Palliative Care, Cancer Center, and embedded primary care clinics). Coordinate with interdisciplinary team members - each team has an NP, licensed clinical social worker, RN, chaplain, and care coordinator Facilitate access to appropriate supportive care services. Coordinate care with inpatient palliative care colleagues, referring providers, outside agencies and local hospices. Clear referral criteria (i.e., no chronic pain management) Assist in the development and growth of our outpatient program by means of quality improvement projects and educational outreach Qualifications Doctor of Medicine or Osteopathy Subspecialty Board certified or eligible in Hospice/Palliative Medicine. Current California state medical license and in good standing with state medical board. It does not include incentives and benefits. The base compensation rate may be adjusted based on training, experience, community need and other possible determining components.
