13 Weeks Position Overview: We are seeking a highly skilled
Registered Nurse (RN) Case Manager with strong Utilization Review
and InterQual experience to join our care management team. This
role plays a vital part in ensuring patients receive the most
appropriate level of care while optimizing hospital resources and
supporting quality outcomes. The ideal candidate will bring a deep
understanding of acute care processes , demonstrate proficiency in
EPIC EMR , and possess the ability to assess patient needs,
coordinate interdisciplinary care, and apply evidence-based
utilization criteria. This position offers an opportunity to
contribute directly to patient advocacy, care coordination, and
healthcare efficiency in a dynamic hospital environment. Key
Responsibilities: Perform initial and concurrent patient
assessments to determine care needs and discharge readiness.
Utilize InterQual criteria to conduct comprehensive Utilization
Reviews ensuring medical necessity and compliance with payer
requirements. Collaborate effectively with physicians, nurses,
social workers, and other healthcare professionals to develop and
implement individualized care plans. Document all case management
activities accurately and promptly within EPIC EMR . Participate in
multidisciplinary rounds and discharge planning discussions to
ensure safe, timely transitions of care. Maintain compliance with
hospital standards, regulatory guidelines, and best practices in
utilization management. Provide coverage during holidays as
required. Requirements Qualifications & Requirements: Active
California RN License (must be current and unrestricted). Minimum
of 2 years’ experience in acute care and case management. Strong
Utilization Review experience using InterQual (required).
Proficiency in EPIC EMR documentation (required). Current BLS/BCLS
certification . Exceptional communication, analytical, and
organizational skills. Demonstrated ability to work independently
and collaboratively in a fast-paced healthcare environment.
